Our office develops the plan of care and schedules in consultation with the client, family, discharge planner, VNA or Hospice provider, and medical professionals where applicable. Our services are tailored to meet your individual needs.
  • We offer a wide variety of services home health services ranging from once-a-week shifts to 24 hourly home care. We provide short-term home care, long-term home care, hourly care, live-in care, homemaking, companionship, transportation and more.
  • Only screened, tested, and/or certified individuals become a part of our team of caregivers. Our office closely monitors their performance and each caregiver is bonded and insured for your protection.

Growing Up

Lisa helped her elderly neighbor, who had an awful case rheumatoid arthritis, with whatever she needed. Her neighbor became her mentor, and helped develop in her a love for helping the elderly. Lisa went on to get married and raise a family. She eventually went back to school for CNA certification and nursing. When her mother fell ill, Lisa and her sisters cared for her. These experiences cultivated a passion to see quality in-home health care services provided to the senior population so they could remain independent in the comfort of their own homes. Thus Golden Horizons was born in Madison Connecticut.

Golden Horizons ElderCare Services, Inc. began as a small in-home health care agency with the goal of high quality home care and customer service, individualized care, and hands-on cooperation between the owner, staff, and clients.  Although we have grown over the years, we continue our commitment to providing quality care on a personal level.

Our aim is to provide high quality home health care services to our seniors and allow them to remain independent in their own homes as they so fervently desire.
